Please fill the camper health history form and bring it in on your first day of camp. All campers are required to have one health history form filled out per summer. You only need to fill out the Medication Administration Authorization Form if your child needs to have medication administered by staff during the camp day.
This form will be filed in secure cabinet and will only be accessible to the camp director, health supervisor, and staff to provide a safe environment for your child.
