Waiver Form
Below is our Waiver/Roster form. The document is in a PDF file format. Please contact [email protected] if you have trouble opening the file.
Please have the parents sign and bring it to the facility on the day of your first game. We will have copies available at our waiver table the day of your first game, however the waiver process will be much faster if you prepare it ahead of time. If you need more roster spots, just print 2 copies and continue your roster on the 2nd copy.
If you are registering as an individual you can sign your team's waiver on the day of your first game. They will also be distributing your uniform shirts at the waiver table in the lobby.
